 Healthy People 2020

 

Purpose of the Assignment

Purpose:  The purpose of this is to involve students in the comprehensive community needs assessment. Students will develop/utilize tools to identify perceived factors (both negative and positive) that impinge on the population’s health. This information will then be used to develop and implement strategies for health promotion.  Topics have been identified from Healthy People 2020 Objectives. Presentations should be developed around one objective from the Healthy People 2020 focus areas.

 

Student Approach to the Assignment

Together, with my group, we decided to assess community needs and solutions to prevent and reduce heart disease in the Thomas Jefferson Health District. My contribution to the paper was to show how we are able to educate everyone in the community on how residents can improve their heart health by making them aware of signs or symptoms that should be addressed early.

Reason for Inclusion

This assignment was an opportunity for my group to devise ways to combat the rising heart disease rates in Virginia.  My group made the connection that the rate of heart disease in the community  was on the rise due to factors such as obesity, tobacco use, and mental health/substance abuse within the population.  I have posted our final paper analyzing our goal of reaching a heart healthy district.

 Leadership

· Initiates community partnerships to establish health promotion goals and implements strategies to meet those goals

My group decided that we could focus on creating partnerships with schools and community events in order to combat the rising rate of heart disease in Virginia. We proposed a program to foster physical activity and healthy eating for people of all ages through a program called Community Action. This was catered for  at-risk groups for heart disease and would benefit most from interventions.  The paper represents the research of heart disease trends and demographics in Virginia and Thomas Jefferson Health District. .
